A recent survey reveals a significant challenge in the software industry: 43% of AI-generated code changes require manual debugging after deployment, despite passing initial tests. This insight, from Lightrun’s 2026 State of AI-Powered Engineering Report, highlights a critical issue as AI-generated code becomes more prevalent. With major companies like Microsoft and Google reporting that a substantial portion of their code is AI-generated, the findings underscore a growing trust gap in AI’s ability to produce reliable code.

## AI-Generated Code and Industry Challenges

The survey, which included 200 senior site-reliability and DevOps leaders from large enterprises in North America and Europe, paints a concerning picture of AI’s role in software development. Notably, none of the respondents felt confident in deploying AI-generated code without multiple redeployment cycles. This lack of confidence is mirrored in the industry’s struggles, as seen in Amazon’s high-profile outages in March 2026. These incidents, caused by AI-assisted code changes, resulted in significant operational disruptions and financial losses, prompting Amazon to implement stricter code approval processes.

In sectors like finance, where errors can have severe consequences, this reliance on human intuition over AI diagnostics is particularly pronounced.

The findings call for a shift towards better observability tools that can provide real-time insights into code execution. Without addressing this visibility gap, organizations risk compounding instability and losing competitive speed. The industry must develop AI tools capable of not only writing code but also effectively monitoring its execution to ensure reliability and trust.

As AI continues to play a larger role in software development, bridging the visibility gap will be crucial. Organizations that successfully integrate AI with robust monitoring capabilities will be better positioned to harness the full potential of AI-driven innovations.
